Real health conversations reveal ChatGPT safety gaps in Global South
Researchers conducted the first real-world safety audit of ChatGPT health advice using actual user conversations from India, Nigeria, Brazil, and Pakistan, rather than synthetic test cases. Analyzing over 15,000 health-related queries from a consent-based corpus of 202,590 conversations spanning late 2022 through early 2026, the study maps demographic patterns in LLM medical guidance across underrepresented regions. This work directly addresses a critical gap in AI safety evaluation: prior benchmarks relied on clinician-written scenarios that miss how vulnerable populations actually interact with generative models. The findings will inform whether LLMs require region-specific safeguards or architectural changes before deployment in healthcare contexts where bias and unsafe recommendations carry material consequences.68
